    CAP Distribution Form pdf

  • To request a distribution/rollover of your Capital Accumulations Provision (CAP) monies download, complete and submit this form to Office of the President. Only separated employees can request a distribution of their CAP monies.

    Compensatory Time Off (CTO) for Clerical (CX) Unit Employees (Word)

  • Colleagues: CUE has signed a side agreement with the University which will allow clerical unit employees to make an election of CTO vs. overtime pay at the time of hire, or in the month of June, 2005. As in past years, you will find attached a model communication to share with your eligible employees who work overtime. If you have any questions regarding the same, please direct them to Danny Gray at extension 40860.

  • These forms are for Research and Technical Employees to elect whether they wish to be compensated for overtime hours by pay or compensatory time off pursuant to the agreement between the University and University Professional and Technical Employees (UPTE).
  • Dual Employment (Word)

  • Dual employment is the term used to describe overtime worked by an employee in either (a) the employee's home department, but in a different job than the employee's permanent appointment or (b) a department other than the employee's home department. The department requesting the employee's additional overtime work initiated the approval process using this form. Prior approval is required before the dual employment work can begin.

    Employee Development Fee Reimbursement Form (Word)

  • University employees who have completed their probationary period (if one is required) and whose performance is satisfactory or better may be reimbursed for classes taken that are not required by the department head in the performance of regular job duties but are approved for employee development. In most circumstances, the employee will pay for and complete the class before being reimbursed. The form requires the signature of the department head, evidence of satisfactory class completion, and proof of costs incurred when submitting it to the Personnel Services Unit of Campus Human Resources.

  • Replaced Time Certification for Leave of Absence and University Contribution Indicator. Form revised to Leave of Absence Notice, email completed form to your Benefits Analyst.
  • Performance Evaluation (Word)

  • Supervisors and managers may download brief guidelines and a recommended performance appraisal form designed to evaluate the performance of employees in a broad range of job classifications.
  • Preferential Rehire Process for Non-Represented PSS Positions (Part I and II) pdf

  • These forms are to be completed by the PSS Preferential Rehires (Non-Represented Professional and Support Staff ) when they have preference or for which they wish to exercise preference for a particular open PSS position that is posted. They are to complete Part I and send it to the Hiring department along with Part II. The forms along with a resume or application must be received by the department that is making the hiring decision by no later than 5:00 p.m. of the posted closing date.
  • Professional Development Program Application (Word)

  • This application packet contains a brief program description, application and supervisor's assessment form. Employees in PSS classification grades 2-5 are eligible to apply. Applicants can download and submit this packet in April of each year to the Organizational Improvement and Staff Development Unit of Campus Human Resources.
  • Reduced Fee Enrollment Application (Word)

  • A regular status employee who meets the admission requirements of the university is eligible for two-thirds’ reduction of both the University Registration Fee and the Educational Fee when enrolled in regular session courses of up to nine units or three courses per quarter or semester, whichever is greater. Upon acceptance as a student, the employee completes the form and forwards it to the Personnel Services Unit of Campus Human Resources.

  • Departmental administrators shall complete this form before notifying affected employees of any department layoff proposal or reduction in time actions. The department shall submit the completed form to the Personnel Services Unit of Campus Human Resources for review prior to the proposed effective date of the action.

  • Training Class Enrollment Application

  • Apply for Training and Development Program classes by completing this form. Please print clearly or type and be sure to include a departmental recharge number and your supervisor's signature indicating permission to attend and authorization to spend departmental funds.   • UCLA Procedures for Reimbursement for Leaves for Union Business pdf

  • The Agreement between the University of California and AFSCME allows for AFSCME represented employees to be granted a Leave of Absence for Union Business. The Leave of Absence for Union Business Contract Article states that the University will be reimbursed for the employee's salary plus 36% to cover cost of employee's benefits. The UCLA Procedures for Reimbursement for Leaves for Union Business describes the procedure to be followed to obtain the reimbursement.
