New UC Demographic Data Transmittal Form

Effective August 1, 2010, the University of California is required to change the manner in which the race and ethnicity data of employees are collected and reported to the Integrated Postsecondary Education Data System (IPEDS). The changes result from the implementation of new standards for Federal agencies on the collection of racial and ethnic data adopted by the U.S. Department of Education in 2007.

As a result of the new requirements, new employees hired on or after August 1, 2010, must complete the new Demographic Data Transmittal Form (Form U5605) to voluntarily self-identify their race, ethnicity, disabled status and veteran status. The revised form will be available for ordering through KP Corporation and for download under the forms section of At Your Service Online (AYSO).

Systems modifications have been completed and were distributed to all campuses via payroll release in early July 2010.

The following UCLA systems will be updated in time to meet this new requirement:

  • Employee Data Base (EDB)
  • Web New Hire
