Commit to Better Health & Earn a $100 Gift Certificate from StayWell

Do you want to earn a $100 gift certificate and get healthier while doing it? Starting January 5, 2010, you can do just that by completing the StayWell Health Assessment (HA). Faculty, staff, retirees and their spouses or domestic partners who are enrolled in a non-Kaiser medical plan are eligible to complete the StayWell HA and participate in follow-up wellness coaching for which they qualify. Children and other family members (other than spouse/domestic partner) are not eligible for the StayWell program in 2010.

Why Participate?

There are many reasons to complete your confidential StayWell HA, but the most important one is your good health. StayWell offers improved, standardized wellness and individual health-management resources at no charge, and participation is voluntary. The program is designed to help eligible employees and their family members achieve a healthier lifestyle.

With StayWell, you and your family can set realistic goals for the coming year and gain valuable insight on:

  • Health risks you may currently have but don’t know about.
  • Information about topics like weight management, tobacco cessation and cholesterol.
  • Tools to help you make positive health behavior changes.

Those who complete an HA between Jan. 5, 2010, and April 15, 2010, will receive a $100 gift certificate that may be redeemed at a wide variety of recognized retailers. Spouses and qualified domestic partners will receive a $50 gift certificate for taking an HA.

Important Information

StayWell is not available to Kaiser members who currently have access to Kaiser’s fully integrated wellness resources. In addition, the leadership of several bargaining units at UC have opted out of participating in the program for 2010.*
