Improving your health with StayWell just got easier. Now you can register for and access the StayWell website and Health Assessment (HA) by using the last 6 digits of your SSN and your date of birth. Because the StayWell website is personalized for each individual member, the last 6 digits of your SSN must be used to identify you the first time you log into the website. Once you have been identified as eligible to participate, you will be able to create your own unique ID and password for all future logins.

Important information about participation
StayWell is not available to Kaiser members who currently have access to Kaiser’s fully integrated wellness resources. In addition, the leadership of several unions at UC have notified the University that they are choosing not to participate in the StayWell Health Management benefits program on behalf of their UC bargaining unit members.
